Comments:
An unannounced renewal inspection was conducted with the provider beginning at 11:10 AM and ending at 12:49 PM. There were eight children in the care of the provider and assistant. The children's ages were 8 months, 15 months, 2, 2, 3, 4, 4, and 5 years old for a total of 19 points. One infant was observed being held by a caregiver. The children were observed playing with toys while an educational television program was playing. Children were observed eating snack which consisted of whole grain crackers and fruit juice. One child was observed returning to the family day home after school. Eight children's records and three household members' records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent emergency evacuation drill was documented as 4/29/19. The most recent shelter-in-place drill was documented as 2/5/19. The provider and assistant are not Medication Administration Training (MAT) certified and cannot administer medication. The provider and assistant are certified in First Aid and Cardiopulmonary Resuscitation (CPR). Standard #: 22VAC40-111-50-A
Description: Based on a review of documentation and staff interview, the family day home did not have a written records of children in attendance each day. Evidence: There were two children signed in on the written attendance sheet for May 6, 2019 and seven physically present in the family day home.

Plan of Correction: The provider signed in all the children that were missing from the attendance sheet. She stated that they would sign in all children during arrival.

Standard #: 22VAC40-111-240-A
Description: Based on observation, not all of the areas inside the family day home were maintained in a safe condition. Evidence: There was an wall outlet plate in the large day care room that had two screws protruding out. The screws tops were mangled and sharp creating a scratching hazard.

Plan of Correction: The provider stated that she would remove the screws and have new ones installed.
