Comments:
An unannounced monitoring inspection was conducted today from 9:30am-11:30am. There were 11 children (32 points) supervised by 3 direct care staff. The home, 3 staff records, 7 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed taking their morning nap and engaging in free play. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-960-E
Description: Based on observation, 5 of 5 bottles were not labeled with all of the required information. Evidence: 1. 5 of 5 infant bottles were not labeled with the date. 2. 1 of 5 bottles was not labeled with the child's full name.

Plan of Correction: The unlabeled bottle was labeled but the label always comes off when washed because of the type of material. We will add the dates every day.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, 3 of 7 children's records did not contain all of the required information. Evidence: 1. Child #1's (start date: 1/3/2016) record did not contain documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. The most recent documented review of the child's emergency contact information was dated 2/22/2018. 2. Child #2's (start date: 8/16/2018) record did not contain documentation of written authorization for emergency medical care. 3. Child #10's (start date: 4/2/2018) record did not contain documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. The most recent documented review of the child's emergency contact information was dated 4/30/2018.

Plan of Correction: I will have the parents complete and sign today.

Standard #: 22VAC40-111-280
Description: Based on observation, the family day home provider did not ensure that potentially poisonous substances were stored in areas inaccessible to children. Evidence: A bottle of cleaning chemicals was found in an unlocked cabinet underneath the sink in the children's bathroom.

Plan of Correction: Corrected during inspection.
