Comments:
An unannounced monitoring inspection was conducted 11:00pm -1:00pm. Upon arrival there were 5 children in care, totaling 16 points with 1 caregiver. All 5 children's records were reviewed. Children were observed watching television. One of the babies was in a crib and another was observed crawling around the play area with toys. Records for both dogs were reviewed and up-to-date.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a record review the licensee did not ensure documentation of a physical examination for each child within 30 days after the first day of attendance. Evidence: Child #4, enrolled on 02/19/19, did not have documentation of a physical examination.

Plan of Correction: The following corrective action was provided by the licensee during the exit interview: Parent was contacted same day and a copy of the physical was obtained .

Standard #: 22VAC40-111-60-B
Description: Based on a record review, the licensee did not ensure all required information was kept for each child. Evidence: Child #1, #4 and #5 did not have written documentation of one of the two required emergency contacts.

Plan of Correction: The following corrective action was provided by the licensee during the exit interview: Second emergency contacts will be obtained and documented in each child's file.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the licensee did not ensure all areas and furnishings of the home were maintained in a safe condition. Evidence: 1. At the top of the door frame of the day care entrance there were 2 hornets nests with a hornet nesting on one. Children pass underneath to enter the day care. There were also 2 other nests in the window frame next to the entrance. 2. The 3 tiered cubby shelf, which the television sat on top of, not flush to the wall and unsecured.

Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The hornets nests will be removed today and cubby will be secured to the wall.
