Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a modification to the license in order to reflect the new address: 2298 Hitchcock Drive, Reston, Virginia 20191. Discussion was also had on the outdoor area and a propose installation of a backyard fence, a review of the Emergency Response and Preparedness Plan to reflect the new location, and a walk through of the entire home.

Comments:
The following inspection took place between the hours of 9:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH's) new address located in Reston, Virginia. At the start of this unannounced monitoring inspection there were a total of 5 children (12 points) with 1 caregiver. A total of 5 children?s files, 1 staff file, and 3 household member?s files were reviewed. The children were engaged in the following: Story time, breakfast, toy play, song, free play, learning activities, infant feeding, bathroom breaks, hand washing, and other organized activities. Standard #: 22VAC40-111-320-G
Description: Based on observation and interview, a wood burning fireplace and associated chimney was not inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Evidence - 1. On the date of inspection (05/15/2019) a copy of an annual inspection report on the chimney and fireplace was not available for review. 2. The Provider stated (during an interview conducted on the date of inspection) that the fireplace is not in use and there are no plans to use it. A television has been placed in front of it. 3. The Provider also stated that there are plans to seal it shut; closing it off for all future use.

Plan of Correction: It shall be seal shut.

Standard #: 22VAC40-111-430-B
Description: Based on observation, a pressure mounted gate was used at the stair opening to the stairway leading to the second story of the home. Evidence - 1. On the date of inspection (05/15/2019), a pressure mounted gate was observed in use at the bottom of the stairway that leads to the second story of the Family Day Home (FDH). 2. On the date of inspection, there was one child in care under that age of two years (Child #5). 3. Currently, the ground floor was being used as the child care area of the FDH. This was the same area that this pressure mounted gate was located.

Plan of Correction: The gate shall be replaced by a pressure mounted gate.
