Technical Assistance:

Licensing inspector discussed household member records, telephone and child abuse and neglect training with provider.

Comments:

An unannounced monitoring inspection was conducted today from approximately 9am-10:50am. There were 10 children (26 points) in the direct care and supervision of 2 caregivers. A sample of 5 children?s records, 2 caregiver's records and 1 household member's were reviewed. The children were observed singing, dancing, reading coloring and preparing for a mothers day event. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Description:	Based on record review, the provider did not review with the parents the children's file annually. Evidence: The file of Child A was last reviewed 04/25/2018.
Plan of Correction:	Parent will update file today.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, children's files were incomplete. Evidence: 1 of 5 children's records reviewed was missing required information. Child B was missing the employer address of both parents.
Plan of Correction:	Parent will update file today.
Standard #:	22VAC40-111-320-G
Description:	Based on record review, the fireplace and associated chimney was not inspected annually. Evidence: The fireplace was last inspected May 04 2018.
Plan of Correction:	I will set up an appointment and ask that the fireplace is permanently disabled.
