Comments:
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 10 children, totaling 27 points. Children's records, administrative records, emergency preparedness items, the physical plant, children's nutrition, programming, and staff:child interactions were inspected today. The children were observed playing outside, singing songs, enjoying group activities, and preparing for lunch. The children were to be served carrots, fish sticks, rice, apples, and milk for lunch today. This inspection was completed between approximately 10:20am and 11:50am. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records are not complete. Evidence: Child record A does not include the father's work contact information. Child record B does not include the parent's medical insurance information.

Plan of Correction: I will ask both parents for the information.

Standard #: 22VAC40-111-70-A
Description: Based on a review of administrative records, it was determined that there is no documentation showing that two parents have been given all required written enrollment information. Evidence: Child record B and C do not include the parent's signature indicating that each parent has received the provider's handbook with her medication policy information.

Plan of Correction: I will ask each parent to sign the form.

Standard #: 22VAC40-111-320-B
Description: Based on observations, it was determined that one electrical outlet was not covered. Evidence: One outlet in the children's play room area was not covered.

Plan of Correction: I will cover it today.
