Comments:
A complaint investigation was conducted on April 17, 2019 beginning at 2:00 p.m. and ending at 3:30 p.m. with the provider regarding infant feeding, infant sleep and daily activities for infants. One infant was observed sleeping in a crib. One infant was observed awake in an infant bouncer seat. Staff interviews were conducted. Documentation was reviewed. The complaint regarding infant feeding, infant sleep and daily activities for infants could not be substantiated. Description: Based on observation, the provider failed to ensure all children were provided with an individual cot or rest mat for napping. Evidence: One child was observed using a folded up quilt on the floor and a second child was observed using a sleeping bag for nap.

Plan of Correction: two of the kids were not on cots , now I make sure that they are on cots.

Description: Based on observation, the provider failed to ensure that all cots and rest mats were equipped with both a bottom cover and top cover. Evidence: Children were observed during nap time and cots and rest mats did not have bottom covers.

Complaint related: No
Description: Based on observation, the provider failed to ensure her hands were washed as required. Evidence: The provider was observed wiping a child's nose. A short time later, the provider then fed a four month old a bottle without first washing her hands.

Plan of Correction: I washed my hands before touching the bottle and cap , but not washed before using the formula scoop. Now I make sure the hands are washed , both of my assistant and me
