Comments:
An unannounced mandated monitoring inspection was conducted today from 10:00am to 11:30 pm. There were 11 children (28 points) and 3 staff present. Children were observed learning about the letter N, having snack, playing in a bouncy, riding a train toy, playing with cars, and going for a walk. Reviewed 2 children's files, 3 staff files, attendance, fire drill log, first aid kit, cribs and physical plant. Violations of the Standards are recorded on the Violation Notice. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: Child#1 and Child#2 did not have documentation of a physical in their files. Start dates were 4/1/19 and 2/11/19, respectively.

Plan of Correction: I will get the physicals from the parents.

Standard #: 22VAC40-111-70-A
Description: Based on record review, parents were not provided in writing whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia prior to the child's first day. Evidence: Child#1 and Child#2 did not have signed documentation of receipt of Liability insurance prior to child's first day of care. Start dates were 4/1/19 and 2/11/19, respectively.

Plan of Correction: I will give to parents and have them sign.

Standard #: 22VAC40-111-70-B
Description: Based on record review, the provider did not obtain parent's written acknowledgement of the provisions of the family day home's emergency preparedness and response plan. Evidence: Child#1 and Child#2 did not have signed documentation of emergency response plan.
