Comments:
An unannounced monitoring inspection was conducted from 1:30-3:00pm. There were 9 children (28 points) in the direct care and supervision of 3 caregivers. It was nap time, so the children were observed sleeping on their cots or in their crib. The playground, first aid kit, emergency materials, injury reports, evacuation drills, a sample of 5 children's records, 7 staff/household member records, and the home were reviewed. Standard #: 22VAC40-111-830-C
Description: Based on record review and interview, documentation was not maintained of emergency evacuation drills being practiced. Evidence: 1. The drills for February, March, and April 2019 were not documented. The Provider stated that they did practice the drill each month, but they did not have a chance to document it.

Plan of Correction: We will have the Assistant document the drills after they are done each month.

Standard #: 22VAC40-111-180-A
Description: Based on record review, all adult household members did not have a current report of Tuberculosis screening. Evidence: 1. Household Member #2's last documented Tuberculosis screening on file was dated 3/3/2017.

Plan of Correction: We will have the Tuberculosis screening updated by the end of the week.

Standard #: 22VAC40-111-500-D
Description: Based on observation, the outdoor play equipment did not meet all requirements. Evidence: 1. Two swings had S hooks that were not tightly closed.

Plan of Correction: We will have to have someone come and take a look at them. We tried to close them but it was very difficult.
