Technical Assistance:
Provided consultation on nutrition. Family day homes shall schedule snacks and meals so there is a period of at least 1-1.5 hours but no more than three hours between snacks/meals. When the family day home serves meals and snacks, they shall follow the most recent, age appropriate nutritional requirements of the USDA.

Comments:
An announced initial inspection was conducted from 10:00 am - 1:00 pm with the provider. There were two children in care, ranging from three years to four years of age, for a total of four points. The children were observed playing outside with sidewalk chalk and a sand table, washing hands, eating snack, playing with a variety of toys, and having a lesson on colors and shapes. Snack served today was orange slices, goldfish, and water. Two children's files were reviewed. Two caregiver/household member files were reviewed and in compliance. Childcare areas and supplies were observed. The first aid kit, battery operated flashlight and weather band radio were observed and in compliance. The emergency drill log and emergency preparedness plan were reviewed and in compliance. The provider is not Medication Administration Training (MAT) certified and cannot administer medications to children in care. Description: Based on a review of two children's files, it was determined that not all files were complete. Evidence: The file for Child #1 (date of first attendance: 10/6/15) did not have a street address for the second emergency contact, immunization record or results of a physical examination as required. The file for Child #2 (date of first attendance: 7/20/15) did not have a street number for the second emergency contact, written authorization for emergency medical care, policy number for medical insurance, immunization record or results of a physical examination as required. Both children need signed acknowledgement of a corrected "Policy on Administration of Medication" form that states provider will not administer prescription or non prescription medication.

Plan of Correction: Will ask the parents to provide the missing information.
