Thank you for your time and assistance with the unannounced renewal inspection conducted today, May 16, 2019. The on-site inspection was from 11:30a.m. to 1:55p.m. The provider was caring for three children and one resident child. (9 points). There were a total of four non-resident children enrolled in the program. The exception to compliance was noted on the violation notice and the risk rating was listed on the supplemental page. There were five children's records reviewed along with the record of the provider. Two household members records were reviewed for updates. There were no medications on-site or currently being administered, which is the policy of the provider. The children brought their lunches from home, and morning and afternoon snacks are provided by the provider. Some of the items reviewed and observed included: documentation of evacuation and shelter-in-place drills, refrigerator and freezer temperatures, snack menu, attendance record, required posted information, injury reports, behavioral guidance, outdoor play, hand washing and bathroom procedures, diaper changing procedures, lunch, outside story time, and nap time. Standard #: 22VAC40-111-840-A Description: Based on review of one injury report and confirmation from the provider, the provider failed to complete an injury report for one child with all of the required information. Evidence: 1. The record for Child #1 contained an injury report dated 02/06/2019 that did not contain the date and time the parent of the child was notified and the method used for notification. 2. The provider verified that the not all of the required information was contained in the report. Plan of Correction: I had the parent sign the report on the day of the incident and will complete the missing information. For future injury reports, I will make sure all of the required information is completed.
