Comments:
A complaint investigation at the family day home was conducted today in response to a complaint received in the licensing office on 2/25/2019. The investigation included caregiver interview, review of records and observation. Evidence gathered did not support the allegation of the use of forbidden action nd the allegation was determined to be not valid. Also alleged and found to be not valid was failing to protect confidential information regarding children in care. A violation that was not complaint related was cited and can be found on the violation notice. An exit meeting was conducted with the provider prior to closure of the investigation.

Description: Based upon review of children's records and provider interview, the provider has not ensured that records and reports on children are maintained accessible to the department's representative for two years from the date of termination of services for a child. Evidence: 1. The provider stated that child 6 was in attendance around the 2018 holiday season. 2. The provider was unable to provide any records for child 6.

Plan of Correction: The provider responded with the following: All children 's, caregiver and household records will be maintained and available for two years after termination of services for a child, separation from employment or termination of residence for a household member.
