Technical Assistance:
Consultation was given on model VA DSS State forms and sent the provider the Administration of Medication Policy form.

Comments:
An unannounced monitoring visit was conducted from 12:10-1:15. Seven children were present with two adult caregivers for eighteen points which is within ratios for supervision. Observed the end of lunch and transition to rest time, reviewed caregivers background and TB checks, two children's records, physical space, emergency preparation and attendance. Lunch served today was Mac and Cheese, apple sauce, mixed vegetables and milk. No medication was reported to be on-site today. The provider has current first aid and CPR certification as well as Medication Administration Training (MAT). Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-100-A
Description: Based on record review, 2 of 2 children's records did not include documentation of a physical exam within 30 days of attendance. Evidence: Child #1 (start date 5/2018) and Child #2 (start date 8/2018) were missing a physical exam record.

Plan of Correction: Will request a physical record from the parents.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 2 children's records is incomplete. Evidence: Child #1 is missing addresses for both emergency contacts.

Plan of Correction: Will request information from parents at pickup today.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 2 of 2 children's records did not include the Medication Policy Evidence: Child #1 (start date 5/2018) and Child #2 (start date 8/2018) records did not include a signed Administration of Medication Policy for the parents by the first day of attendance.

Plan of Correction: Provider will print the form and have the parents sign at pickup today.

Standard #: 22VAC40-111-90-A
Description: Based on record review, 1 of 2 children's records did not include documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the child may attend the family day home. Evidence: Child #2 had no immunization record in the file and started attending the day care 8/2018.

Plan of Correction: The provider will request information from the parent.
