Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the special exception submitted to the Town of Vienna to increase the Family Day Home (FDH) capacity from 7 and up to a possible 12. If approved the FDH shall move forward in requesting a modification to the posted capacity in order to reflect this newer approved number.

Comments:
The following inspection took place between the hours of 9 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Vienna, Virginia. At the start of this unannounced renewal inspection there were a total of 8 children (to include 1 resident child) (20 points) with 2 caregivers. A total of 7 children?s files, 2 staff files, and 3 household member?s files were reviewed. The children were engaged in the following: circle time, infant sleeping, infant feeding, learning, song, snack, bathroom break, hand washing, diapering, and other organized activities. Standard #: 22VAC40-111-780-B
Description: Based on review, 1 of 7 children's files reviewed indicated that annually, the Provider did not review with each parent the emergency contact information (as required in 22 VAC 40-111-60 B 2) to ensure that the information was correct, and obtain the parent's signed acknowledgment of that review. Evidence - 1. On the date of inspection (05/21/2019), the most current documented review of the information on file to be used in an emergency was dated older than one year for Child #3. 2. The last review for Child #3 was dated - 01/08/2018. 3. Child #3 was in attendance during the inspection conducted 05/21/2019.

Plan of Correction: An updated review shall be conducted and documented.

Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 7 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (05/21/2019), documentation of a physical examination was not available for Child #6. 2. The start date for Child #6 was listed as 04/15/2019. 3. Child #6 was in attendance during the inspection conducted on 05/21/2019.

Plan of Correction: Documentation of a physical examination for Child #6 shall be obtained and placed on file.
