Comments:

An unannounced monitoring inspection was conducted today from 10:00am-11:20am. There were 6 children (15 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, playing with small manipulative toys, and coloring. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice. Description:	Based on review of children's records, 1 of 5 children did not have documentation of a physical examination within 30 days of the child's first day of attendance Evidence: Child#1's record did not contain a physical examination within 30 days of the child's first day of attendance. The child's first day of attendance was documented as 4/8/2019.
Plan of Correction:	We will ask the parents to bring us the physical form.
