Comments:
An unannounced renewal inspection was conducted today from 10:45am-12:45pm. There were 10 children (32 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, learning numbers in Spanish and English, being bottle fed, playing outside, and eating snack. Diapering procedures and hand washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not maintain complete children's records. Evidence: Child#1's record is missing the address for one emergency contact. Child#2's record is missing the child's medical insurance policy number. Child#3's record is missing the child's medical insurance policy number. Child#4's record is missing the father's place of employment, a work telephone number and 1 of 2 required emergency contacts.

Plan of Correction: I will contact the parents to ask them for the information.

Standard #: 22VAC40-111-70-A
Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance. Evidence: Child#3's record did not have written documentation of the provider's liability insurance declaration.

Plan of Correction: I will ask the parent to sign the form to add it to the child's file.
