Comments:
An unannounced monitoring visit was conducted today. Seven children were present with three caregivers for twenty five points which is within ratios for supervision. Observed children in free play, diapering procedures, infant care and reading. Lunch to be served today is Vienna sausages, rice with mixed vegetables and apple sauce. Emergency preparation is current. No medication was reported to be on-site today. Reviewed two children's records, trainings for care givers, physical space inside on both levels as well as outside driveway area where children play. Standard #: 22VAC40-111-60-B
Description: Based on record review, 2 of 2 children's records are incomplete. Evidence: Child #1 is missing a second emergency contact and parents signature for emergency authorization. Child #2 is missing an emergency contact address

Plan of Correction: Spoke to parents on 4/25/2019. Parents provided additional information immediately and children's files have been updated.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 2 of 2 children's are missing complete Information For Parents. Evidence: Child #1 and Child #2 are missing a completed Provision for Emergency Care as the relocation site is blank.

Plan of Correction: Spoke to parents on 4/25/2019. Parents provided additional information immediately and children's files have been updated.

Standard #: 22VAC40-111-80-A
Description: Based on record review, one of two children's records did not contain proof of identity within seven business days. Evidence: Child #1's first day of attendance was 4/1/2019 and did not have documentation that proof of identity had been verified by the provider.

Plan of Correction: Spoke to parents on 4/25/2019. Parent has applied for birth certificate and will provide immediately upon receipt.

Standard #: 22VAC40-111-90-A
Description: Based on record review, one of two children did not have documentation of being adequately immunized before attending the day care. Evidence: Child #1 started on 4/1/2019 and did not have an immunization record on-site.

Plan of Correction: Spoke to parents on 4/25/2019. Parent provided additional document immediately and child's file has been updated.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas of the home are not maintained in a clean, safe or operable condition. Evidence: 1. Three cribs have become very loose-the sides and end panels are not tight from being pulled and pushed apart. 2. A screw is protruding from one crib

Plan of Correction: Items noted do not pertain to cleanliness of day care. Day care has a daily and weekly cleaning schedule. We also conduct periodic deep cleaning for inside and outside of day care including but not limited to professional carpet cleaning, sanitation and cleanliness. 1. All three cribs have been reviewed and panels tightened. 2. Minor protruding screw o the outside of the crib has been tightened.
