Comments:
An unannounced monitoring inspection was conducted May 15 2019 between 11:15 and 12:30pm. There were 4 children in the direct care of the provider. Children were observed being fed, washing hands, and napping. The family day home is clean and organized. The play room offers plenty of materials and toys for the children. Standard #: 22VAC40-111-830-C
Description: Based on observation and provider interview, documentation for emergency evacuation and shelter-in-place drills were not documented. Evidence: The provider stated that drills were conducted but not written down.

Plan of Correction: I will write them down moving forward.

Standard #: 22VAC40-111-100-A
Description: Based on record review, 1 of 5 children's records reviewed did not have documentation of a physical examination by or under the direction of a physician. Evidence: Child #4 record does not have a physical examination on file.

Plan of Correction: I will talk to her mom today.

Standard #: 22VAC40-111-70-A
Description: Based on record review 1 of 5 children's records reviewed did not have documentation that the forms required for parents were given prior to the first date of attendance. Evidence: The following forms are in Child #1 record, but not signed by the parents. 1) Medication Adminitration Form, 2) Information for Parents, 3) Liability insurance form

Plan of Correction: I will give them the forms when they pick him up.

Standard #: 22VAC40-111-90-C
Description: Based on record review, 1 of 5 children's records reviewed did not have documentation of additional immunizations for a child every six months for children under the age of 2 years. Evidence: Child #4 record last documented immunization record is dated 9/10/18

Plan of Correction: I will talk to her mom today.
