Comments:

An unannounced monitoring inspection was conducted today from 11:00am-1:00pm. There were 7 children (18 points) directly supervised by 2 staff. The home, 2 staff records, 3 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, building with blocks, pretend play cooking and eating lunch. Lunch served today was strawberries, chicken nuggets, chocolate sandwich and milk. Preschool age children were able to serve themselves. Infants were given individual bowls of food and encouraged to eat independently. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #:	22VAC40-111-120-B
Description:	Based on record review, the provider did not maintain complete staff records. Evidence: Caregiver 2 is missing an emergency contact in their record.
Plan of Correction:	Today, I will get the name and phone number for the emergency contact.
Standard #:	22VAC40-111-200-A
Description:	Based on record review, the provider did not orient the the assistant by the end of their first week of assuming job responsibilities. Evidence: Caregiver 2 did not yet complete an orientation. (Caregiver 2, start date: 5/1/19)
Plan of Correction:	I will do today.
Standard #:	22VAC40-111-560-A
Description:	Based on observation, the provider failed to provide direct care and supervision of each child at all times. Evidence: For approximately 15 minutes, Caregiver 1 was upstairs preparing lunch for the children. During this time, Caregiver 2 was alone with 7 children (18 pts). Caregiver 2 entered the bathroom to wash children's hands before lunch. While Caregiver 2 was in the bathroom, Child 1, 2, and 3 climbed onto the top of a table. Child 1 pushed Child 2 causing them to cry and almost fall off the table.
Plan of Correction:	Next time I will not leave the children alone.

Description:	Based on record review, the provider did not ensure all staff submitted to fingerprinting. Evidence: Caregiver 2 has a completed fingerprint result from a different family day home provider on file. A fingerprint based criminal history check was not conducted for the current provider's program. (Caregiver 2, start date 5/1/19)
