A Subsidy Health and Safety inspection was conducted on May 17, 2019 from 11:00am to 1:00pm to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-160-B Description: Based on record review, the provider did not maintain a written hard copy of arrival and departure of each child as it occurs. Evidence: The provider did not have documentation of departure of each child as it occurred. The attendance sheet for the week of May 13, 2019 did not account for the departure of each child. Plan of Correction: I will make sure to write down when the child leaves.
