Comments:
An unannounced monitoring inspection was conducted today from 9:30am-10:30am. There were 11 children (31 points) directly supervised by 3 staff. The home, 3 staff records, 4 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, building with blocks, cleaning up, and playing outside. There was a supply of books, toys, and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider did not obtain an updated immunization record once every six months for children under the age of two years. Evidence: Child 1 has an immunization record dated 4/20/2018. Child was born on 2/21/2018 and was enrolled in the program 5/14/2018.

Plan of Correction: I have reached out to the parent to request the updated immunization record.
