Comments:
An unannounced monitoring inspection was conducted this date. When the Inspector arrived, there were a total of 12 children (24pts), with 1 Staff and 1 Volunteer who is not counted in ratio. Provider exceeded the maximum number of points per caregiver for 15 minutes. The Provider arrived within 15 minutes. The children were observed playing on the newly installed playground. Ample amounts of space on padded mats, patio and grass to play on. The staff were observed actively engaged with the children, encouraging them to climb, kick balls and assisted with them on swings. Artwork was drying when the Inspector arrived. The children painted Suns and flowers during this morning's class time. Observation of the crib room, the programming area and bathroom was completed. No medications or food allergies at this time. A total of 5 children's records and all staff records were reviewed. This inspection began at approximately 10am and ended at 12:20pm. Description: Based on review of staff records, First Aid certification was not maintained. Evidence: Staff #1's First Aid certification expired in April 2019.

Description: Based on review of staff records, CPR certification was not maintained. Evidence: Staff #1's CPR certification expired in April 2019.

Description: Based on observation of the playground, playground equipment was not anchored securely with ground supports. Evidence: The new playground equipment installed over rubber resilient surfacing is not anchored into the ground.

Plan of Correction: I will have a contractor anchor the equipment where it will remain stationary.

Standard #: 22VAC40-111-570-A
Description: Based on observation of the children in care, the provider failed to ensure that the caregiver did not exceed 16 points. Evidence: When the Inspector arrived, the provider was not home. Staff #2 and 1 Volunteer were observed in the background with the children playing. 12 children were present(24pts). The provider returned within 15 minutes.

Plan of Correction: I did not realize that volunteers could not with points. I will not leave the staff with more than 16 pts.
