Technical Assistance:
Today's listed menu does not match what the children told the inspector they ate. Provider was reminded to update the menu to match what was served before the end of the day.

Comments:
Today an unannounced Renewal inspection took place between 1:50pm and 3:45pm. There was 1 child present with the provider for a total of 2 points within the supervision guidelines. The child was observed reading books, singing songs, talking about body parts (counting eyes, ears, fingers and toes) and building towers with lego blocks. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-180-A
Description: Based on record review and interview, the provider did not obtain a current Report of Tuberculosis (TB) Screening form every two years from the date of the first screening. Evidence: Household member #1's most recent TB screening is dated 5/16/2017. The provider stated that she went to health department for her test but doesn't know where the results are.

Plan of Correction: I will look for the results and if I can't find it, we will go back to have them print the results again.

Standard #: 22VAC40-111-240-A
Description: Based on observation, furnishings of the family day home outside were not maintained in a safe and operable condition. Evidence: There is a giraffe riding toy in the backyard with a broken handlebar. The entire right handlebar is missing and there is jagged plastic on the toy where the handlebar used to be.

Plan of Correction: I will throw it in the trash in the backyard.
