Technical Assistance:
Licensing inspector discussed child abuse and neglect training with provider.

Comments:
An unannounced renewal inspection was conducted today from approximately 9:30am-12:30pm. There were 3 children (10 points) directly supervised by 1 caregiver. A sample of 3 children?s records, 1 caregiver's record and 3 household member's records were reviewed. The children were observed playing on the deck, eating, and playing in the playroom. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-111-940-B
Description: Based on interview, the provider did not have an operable thermometer to monitor the freezer compartment temperatures. Evidence: When asked if there was a freezer thermometer the provider stated I don't have one.

Plan of Correction: I will get one tomorrow.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were incomplete. Evidence: 2 of 3 children's records reviewed were missing required information. 1. Child A was missing father's employer phone number, insurance policy number, physicians phone number, and medication authorization form. 2. Child B was missing medication authorization form.

Plan of Correction: For Child A I will check today and for Child B I will do it today.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas and furnishings inside the house were not kept in a clean safe and operable condition. Evidence: 1. On the wall in the main play area the paint was peeling. 2. The top of 2 plastic 3 drawer shelves located in the main play area were broken.

Plan of Correction: I am going to ask the guy to fix it.

Standard #: 22VAC40-111-340-C
Description: Based on observation, the bathroom was missing required contents. Evidence: The bathroom located on the entry level near the main play area was missing paper towels.

Plan of Correction: I will put it there today.

Standard #: 22VAC40-111-530-G
Description: Based on observation, a child under two used a pillow. Evidence: 1. Provider placed Child A, who is under the age of two, in a crib. 2. The crib had a pillow and a stuffed animal inside.

Plan of Correction: I will remove the pillow and stuffed animal from the crib today.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on observation, the provider did not complete a central registry search for a household member 14 years of age or older. Evidence: Household Member A did not have a complete central registry search on file.

Plan of Correction: I will fill out the form this week.
