Comments:
A Subsidy Health and Safety inspection was conducted on 05/21/2019 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 12:40pm-4pm. There were 11 children (23 points) in the direct care and supervision of 2 caregivers. A sample of 5 children?s records, 2 caregiver's records, 2 household members records and 1 pet record were reviewed. The children were observed eating lunch and napping. Changes are being made to the amount of household members in the home. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on interview, the provider did not keep a written record of daily attendance. Evidence: There was no record of daily attendance. The provider stated I get the same kids everyday.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-665-400-B-3
Description: Based on observation, the emergency evacuation and relocation plan was incomplete. Evidence: The evacuation and relocation plan was missing procedures to reunite parents with children after the emergency.

Description: Based on observation, the emergency shelter in place plan was incomplete. Evidence: The shelter in place plan was missing procedures to reunite parents with children after the emergency.

Description: Based on observation, the emergency lockdown plan was incomplete. Evidence: The lockdown plan was missing procedures to reunite parents with children after the emergency.

Description: Based on record review, lockdown procedures were not practiced at least annually. Evidence: Lockdown procedures were not practiced in 2018. There was no documentation of lockdown procedures.

Plan of Correction: I will do my lockdown drill in July.
