Comments:
An Unannounced Renewal Inspection was conducted today. There were 2 children (6 points) directly supervised by 1 caregiver. The home, 1 caregiver's records, 3 children's records, 2 household member's records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping or playing freely in the playroom. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on review, the provider failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 3 children's records and found that: 1) Child 1 did not have documentation of emergency medical authorization or medical insurance policy numbers. 2) Child 3 did not have documentation of an address for one of their emergency contacts.

Plan of Correction: Will ask the parents for the information.
