Discussion was held on the topics of current enrollment, current staffing, and the possible use of substitutes in the future. The Provider was reminded that any substitute would need to have background checks attributed to her Family Day Home (FDH) and its facility identification number (as in the case of the fingerprint check). Background checks obtained in the name of a different provider are not interchangeable and cannot be used by a different FDH. Discussion that the FDH has recently been approved by Fairfax County to increase the capacity to 12. The paper shall be sent in to the Division of Licensing once it has been mailed to the FDH.
The following inspection took place between the hours of 8:30 am through 12:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Centreville, Virginia. At the start of this unannounced renewal inspection there were a total of 6 children (15 points) with 2 caregivers (an additional child arrived later). A total of 6 children?s files, 2 staff files, and 3 household member?s files were reviewed. The children were engaged in the following: games, large motor skills outside in the backyard, music, learning, lunch, hand washing, diapering, infant feeding, nap time and other organized activities. Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 6 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (05/22/2019), documentation of a physical examination was not available for review for Child #4. 2. The start date for Child #4 was listed as 03/04/2019. 3. Child #4 was in attendance during the inspection conducted on 05/22/2019. Plan of Correction: Documentation of a physical examination shall be obtained and placed on file for Child #4. The Parent has been reminded and will continue to work with the parent to get this required documentation.
