Comments:
A Subsidy Health and Safety (SHSI) inspection was conducted at the Perera FDH located in Centreville, Virginia. This inspection was conducted on 05/22/2019, between the hours of 8:30 am through 12:30 pm to review supplemental health and safety requirements. An Unannounced Renewal Inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that were not covered in licensing standards were reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and were identified on the violation notice. If you should have any questions, please call or e-mail. Thank you. Charlie Perkins, Licensing Inspector (703) 309-3963

Violations:
Standard #: 22VAC40-665-400-B-7
Description: Based on review, the current Emergency Preparedness and Response Plan (EP&RP) does not list the continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence - 1. On the date of inspection (05/22/2019), the EP&RP does not list these procedures for the regulation that went into effect on October 17, 2018. 2. The Family Day Home has a plan that addresses evacuation, shelter-in-place, relocation, and lockdown.

Plan of Correction: Will add these new procedures.
