Description: Based on review of four children's records, one record did not contain documentation of a physical within thirty days after the first day of attendance. Evidence: The record for Child #4 (start date 9/20/18) did not contain documentation of a physical.

Plan of Correction: Provider will obtain the required information from the child's parent.

Standard #: 22VAC40-111-60-A
Description: Based on review of four children's records, three records did not contain the required information. Evidence: The records for Child #1 (start date 7/27/18), Child #3 (start date 9/20/18) and Child #4 (start date 9/20/18) did not contain complete insurance information.
