Technical Assistance:
We discussed the requirements for playground equipment and surface in fall zones. We reviewed the requirements for out-of-state child abuse registry checks.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:35 AM to 2:00 PM. Today, there were nine children, ages infancy through four years (22 points), with two caregivers. I viewed daily routines, staffing, supervision, interactions with children, equipment, activities, posted information, annual review of the emergency plan, emergency supplies and drill records, pet record, five records for children, four caregiver/resident records, caregiver training, indoor and outdoor areas. There was not any medication present this date. The children have been provided a variety of age appropriate toys, books and materials. The group discussions included learning about colors and bumble bees. The children enjoyed active play outside. The caregivers were observed providing frequent interactions with the children and appropriate supervision. Let me know if you need any assistance. 540/430-9259

Standard #: 22VAC40-111-500-C
Description: Based on observation of the outdoor playground equipment, and an interview with the provider, the provider did not ensure that outdoor equipment was not over hard surfaces where children may fall. Evidence: One Little Tikes composite structure had hard packed dirt at the exit area of a red slide, with a height of two feet nine inches and the fall zone of a beige ladder with a height of two feet. A second Little Tikes composite structure had hard packed dirt at the fall zones of a purple ladder with a height of two feet. The licensing inspector was told that there were mats in these areas which became worn and were removed.

Plan of Correction: The provider will replace the mats that have been used at fall zones of equipment.
