An unannounced monitoring inspection was conducted today between the hours of 9:00 am through 10:00. There were a total of 7 children (26 points) with 2 caregivers. A total of 2 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, shapes, and nap time. Standard #: 22VAC40-111-100-A Description: Based on a review of children records, the provider has not obtained a physical examination within 3 days of the first day of attendance. Evidence: Child #1, date of attendance 1/28/19 does not have documentation of a physical examination. Plan of Correction: I messaged the parent for a copy of the physical examination.
