Technical Assistance:
There is one crib in use that meets the requirements, another crib (Sniglar) is stored away and not in use.

Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:00 am through 10:15. There were a total of 3 children (9 points) with 1 caregiver. A total of 5 children's files were reviewed. No changes to the caregiver/household records. The children were engaged in the following: breakfast, blocks, coloring dress up and manipulatives. Standard #: 22VAC40-111-60-B
Description: Based on a review of children records, the provider has not obtained all the required information for each child. Evidence: Child #1, date of first attendance 5/20/19 does not have documentation of medical insurance policy number and 2 emergency addresses.

Plan of Correction: I will ask the parent for this information.
