Comments:

An unannounced renewal inspection was conducted on 05/24/2019 from 12:40pm - 1:40pm. At the time of entrance there were four children in care with the provider. The ages of the children counted for a total of eight points. Children were observed during nap. Records reviewed for the four children in care, the provider and one household member. The entire home and backyard play area was additionally reviewed.

Description:	Based on observation, the first aid kit used for field trips lacked the required elements. Evidence: 1. The first aid kit the provider took on a walking field trip to the neighborhood park lacked the following required items: a. Digital thermometer; b. Triangular bandages; c. Single use gloves such a surgical or examination; d. An ice pack or cooling agent.
Plan of Correction:	All missing supplies will be obtained and placed in the field trip first aid kit before the FDH resumes operation on Tuesday, May 28th.
