An unannounced monitoring inspection was conducted today from 11:30am-1:15pm. There were 2 children (4 points) directly supervised by the provider. The home, 2 staff records, 2 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, doing puzzles, listening to stories, and eating lunch. Today's lunch consisted of meatloaf, peas, rice, apple sauce and milk. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: Child#1's record is missing the street number for 1 of 2 emergency contacts. Child#2's record is missing 1 of 2 required emergency contacts, the address for the second emergency contact, and the authorization for emergency medical care. Plan of Correction: We will request the missing information.
Standard #: 22VAC40-111-70-A Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance. Evidence: Child#2's record is missing provisions of the family day home's emergency preparedness and response plan. Plan of Correction: We will have the parents sign the form.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not obtain a tuberculosis (TB) screening/test every two years from the date of the last TB screening/test. Evidence: Household member#2's record contains a TB screening/test that was last updated on 9/21/2016. Plan of Correction: We will try to locate it, or we will obtain a new record.
