Standard #: 22VAC40-111-60-A Description: Based on a review of children's records, 4 out of 4 records were incomplete. Evidence: 1. Child #1's record was missing the required annual update of information. The last update was on 1/17/2018. 2. Child #2's record was missing the required annual update of information. The last update was on 1/8/2018. 3. Child #3's record was missing the required annual update of information. The last update was on 12/14/2017. 4. Child #4's record was missing the required annual update of information. The last update was on 9/19/2017. Plan of Correction: I will ask all of the parents to update their information today when they pick-up.
Standard #: 22VAC40-111-570-A Description: Based on observation, the Provider exceeded the allowable points per adult caregiver number. Evidence: 1. There were 20 points present based on the children's ages with one adult caregiver. There were 3 children present (Child #2, 4, & 6) under 16 months of age (12 points) and 4 children (Child #1, 3, 5, & 7) between 2-4 years of age (8 points). Plan of Correction: I will make sure that we always have 1 caregiver for every 16 points. My Assistant was here but went to a Doctor's appointment during the rest period.
