Comments:
An unannounced Renewal inspection was conducted today from 12:15pm-2:15pm. There were 7 children (20 points) in the direct care and supervision of one caregiver. A sample of 4 children's records, 4 caregiver/household member records, the first aid kit, emergency materials, the emergency plan, the outdoor space, and the home were reviewed. It was nap time so the children were observed laying down on their cots and falling asleep. The day care space was organized and there was an abundance of age appropriate materials available for the children. The Provider is still in the process of obtaining approval from zoning to move her day care to the basement area. If you have any questions, please contact me at shahana.green@dss.virginia.gov .

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, 4 out of 4 records were incomplete. Evidence: 1. Child #1's record was missing the required annual update of information. The last update was on 1/17/2018. 2. Child #2's record was missing the required annual update of information. The last update was on 1/8/2018. 3. Child #3's record was missing the required annual update of information. The last update was on 12/14/2017. 4. Child #4's record was missing the required annual update of information. The last update was on 9/19/2017.

Plan of Correction: I will ask all of the parents to update their information today when they pick-up.

Standard #: 22VAC40-111-570-A
Description: Based on observation, the Provider exceeded the allowable points per adult caregiver number. Evidence: 1. There were 20 points present based on the children's ages with one adult caregiver. There were 3 children present (Child #2, 4, & 6) under 16 months of age (12 points) and 4 children (Child #1, 3, 5, & 7) between 2-4 years of age (8 points).

Plan of Correction: I will make sure that we always have 1 caregiver for every 16 points. My Assistant was here but went to a Doctor's appointment during the rest period.
