Comments:
An unannounced monitoring inspection was completed on May 16, 2019 from approximately 3:45p.m. to 5:30p.m. Two children were on care for a total of five points, with two caregivers. Staff and children were observed in the indoor space engaged in activities to include free play. Interviews and interactions were conducted and five children?s records, three caregiver records and two household member records were reviewed. There are no children currently on medication. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left. Violations:
Standard #: 22VAC40-111-940-A
Description: Based on observation, the licensee failed to ensure that temperatures shall be maintained at or below 40 degrees Fahrenheit in refrigerator compartments. Evidence: 1. The refrigerator temperature was observed at 52 degrees Fahrenheit. 2. Caregiver #1 acknowledged the temperature was too high.

Plan of Correction: The provider will adjust the temperature in the refrigerator.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver records, the licensee failed to ensure that each caregiver and adult household member obtain a tuberculosis screening every two years for two of two household members and two of three caregivers. Evidence: 1. HHM#1, HHM#2 and Caregiver #3's records contained tuberculosis screenings/tests dated February 10, 2017 and April 22, 2019 and therefore not within the required two year time frame. Caregiver #2's record continued tuberculosis screenings/tests dated February 22, 2016 and April 4, 2018 and therefore not within the required two year time frame. 2. Caregiver #1 acknowledged these screenings were not obtained timely.

Plan of Correction: The provider will required all caregivers and household members to get updated TB screenings/tests ahead of time.
