Comments:
An unannounced monitoring inspection was conducted on May 21, 2019 from approximately 11:30 am to 1:00 pm. Provider was present with five children for a total of twelve points. Five children's records were reviewed. The records for the provider and adult household member were also reviewed. The providers responses for the Description of Action To Be Taken were not received as of May 30, 2019 and will not appear on this Violation Notice.

Description: Based on interview, provider did not ensure there is a written emergency preparedness and response plan that included all required components. Evidence: Provider could not provide the written emergency preparedness plan during inspection. Provider stated it had been written but could not be located.

Description: Based on observation, provider did not ensure infant walkers are not used. Evidence: Child #5 was observed sitting in and rolling around the room in an infant walker.

Description: Based on observation and interview, provider did not ensure that when toilet chairs are used that they are emptied promptly, cleaned and sanitized after each use. Evidence: A potty chair was observed in the children's bathroom and there was yellow liquid in the bowl. Provider confirmed that a child was in the process of potty training and he had gone to the bathroom earlier.
