Description:	Based on observation, the provider failed to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. At 9:35 am eight children were in attendance and the times of arrival were documented on the attendance sheet posted on the wall by the door. At approximately 10:45 am four more children arrived. At 11:45 am the four children that arrived at 10:45 am had not been checked in on the attendance sheet. 2. The provider stated it is up to the parents to check their children in on the attendance sheet.
Plan of Correction:	A sign will be put by the door to remind parents to sign their children in and out everyday.

Description:	Based on interview, the provider failed to ensure all staff completed within 90 days of employment Virginia Preservice Training for Child Care Staff sponsored by the Department of Social Services. Evidence: The hire date for staff 2 was 3/26/18. The provider and staff 2 stated staff 2 has not completed preservice training.
Plan of Correction:	Staff 2 will be required to complete Virginia Preservice Training for Child Care Staff. In the future all new staff will be required to complete Virginia Preservice Training for Child Care Staff within the first 90 days of employment.

Description:	Based on interview, the provider failed to ensure one caregiver was certified in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within the first 90 days of employment. Evidence: The provider and staff 2 stated staff 2 was not currently certified in CPR but is scheduled to complete the training Saturday.
Plan of Correction:	Staff 2 is scheduled to complete CPR training on Saturday. In the future all new staff will be required to complete CPR training within the first 90 days of employment.

Evidence: The provider stated she has not made updates to the emergency plan to include the required update.
Plan of Correction:	The emergency plan will be updated to include the new requirement.

Description:	Based on observation and interview, the provider failed to maintain a record of the dates of the lockdown practice drills for one year. Evidence: 1. The provider post the documentation of the practice drills. The posted document did not contain information on a lockdown drill be conducted in the last year. 2. The provider stated they have completed a lockdown drill but she must not have documented the drill.
Plan of Correction:	All emergency drills will be documented and kept for a year in the future.
