An unannounced renewal inspection was conducted May 28 2019 between 11:15am and 1:00pm. There were 7 children in the direct care of the provider and 2 assistants. Children were observed singing, reading stories, being fed, and being diapered. The family day home is clean and organized. The home offers age appropriate materials. Standard #: 22VAC40-111-170-A Description: Based on record review, 3 of 3 staff member records did not contain documentation of report of Tuberculosis Screening prior to coming into contact with children. Evidence: Staff #1 record does not have documentation of a tuberculosis screening. Staff #2 tuberculosis screening is outside of the allotted time frame allowable prior to hire. Staff #3 tuberculosis screening is outside the allotted time frame allowable prior to hire. Plan of Correction: I will have the staff setup appointments for the doctor.
Standard #: 22VAC40-191-40-D-1-b Description: Based on record review, 2 of 3 assistant records do not have completed central registry searches within 30 days of the employment at the facility ends. Evidence: Staff #2 has a copy of the sent central registry search, but not the completed documentation. Staff member began employment during the 2nd week of April 2019. Staff #3 record does not contain documentation of a central registry search. Staff #3 began part time employment during the 2nd week of April 2019 Plan of Correction: I will call them today during lunchtime.
