Technical Assistance:
All background checks verified for household members to process renewal application once received.

Comments:
An unannounced renewal inspection was conducted today from 12:45pm-2:00pm. A total of 5 children [13 points] were present with 3 caregivers. Children were observed during naptime, toileting, handwashing and early departure. A sample of 3 children's records, 3 caregiver records, emergency plan, emergency supplies, and emergency drills were reviewed. There were no medications on site. Standard #: 22VAC40-111-120-B
Description: Based on records reviewed and interview, 1 out of 2 caregivers' records did not include the required information. Evidence: The record for Caregiver #2 was missing the name and phone number of an emergency contact and documentation of at least two references of character and reputation prior to employment. Caregiver #1 reported that Caregiver #2 began trial employment two weeks prior to the inspection.

Plan of Correction: The caregiver is going through a trial period. I will obtain all of this information if and once the caregiver is hired.

Standard #: 22VAC40-111-200-A
Description: Based on records reviewed and interview, 1 out of 2 caregiver records did not include documentation of orientation training within the first week of assuming job responsibilities. Evidence: The record for Caregiver #2 was missing documentation of orientation training. Caregiver #1 reported that Caregiver #2 began employment two weeks prior to the inspection.

Plan of Correction: The caregiver is going through a trial period and I will have the caregiver complete the orientation training if hired.
