Technical Assistance:
Technical assistance was provided regarding equipment in the home.

Comments:
An unannounced monitoring inspection was conducted today from 10:00am-12:00pm. There were 7 children (18 points) including the provider's two children directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, building with blocks, and eating snack. Diapering procedures were also observed. There was a supply of books, toys, and materials for the children. Standard #: 22VAC40-111-810
Description: Based on record review, evacuation and relocation procedures did not include all required information. Evidence: The evacuation and relocation procedures did not include method of alerting caregivers and children of the emergency, written primary and secondary routes out of home, the assembly point outside of the home, method of ensuring all children are accounted for, and method of ensuring all emergency supplies are taken during the evacuation.

Plan of Correction: We will add the required information into the procedures.

Standard #: 22VAC40-111-830-A
Description: Based on record review, the provider did not conduct monthly emergency evacuation procedures. Evidence: The provider stated drills were conducted for March and April 2019 but the drills were not documented.

Plan of Correction: We will write down when the drills are done.

Standard #: 22VAC40-111-830-B
Description: Based on record review, the provider did not conduct two shelter in place drills twice a year. Evidence: Two shelter in place drills were not conducted in 2018.

Plan of Correction: We will conduct the drills twice year.

Standard #: 22VAC40-111-920
Description: Based on record review, the provider did not have a current menu available. Evidence: The menu posted did not have current dates.

Plan of Correction: We will write the current dates on the menu.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas outside the family day home were not kept in a safe condition. Evidence: On the outside playground, there were rocks at the bottom of the slide which causes a potential safety hazard to children as they go down the slide.

Plan of Correction: The rocks were cleaned up promptly during inspection and the entire area was safe.

Standard #: 22VAC40-111-250-C
Description: Based on observation, the provider did not remove toys hung from the sides of the crib with cords when the child begins to push up on hands or is five months of age. Evidence: Two toys were observed on two different cribs. The provider stated the cribs were utilized by children over the age of five months.

Plan of Correction: We will remove toys today.
