05/29/2019 Arrived 11:10am Departed 1:30pm An unannounced monitoring inspection was conducted today. Ten children were in care for a total of 26 points with two caregivers. Children's and staff records were reviewed. The children were playing outdoors, eating lunch and playing with toys. Thank you for your cooperation. Violations:
Standard #: 22VAC40-111-450-D
Description: Based on observation the provider failed to ensure that all doors and windows used for ventilation are screened securely. Evidence: 1. During the inspection the exit door remained open, but there is no screen in place.

Plan of Correction: The provider will ensure that the door closed until a screen door can be purchased.

Standard #: 22VAC40-111-560-A
Description: Based on observation, the provider failed to ensure that there is direct care and supervision of each child at all times. Evidence: 1. During the inspection a child who is 14 months old walked out the open door onto the porch for less than one minute. An older child told the provider, "He's outside!"

Plan of Correction: The provider will ensure that the door is closed and that all children are closely supervised.

Standard #: 22VAC40-111-680-A
Description: Based on observation, the provider failed to ensure that her hands are washed with liquid soap and warm running water each time they come into contact with body fluids. Evidence: 1. The provider wiped two children's noses during the inspection. The provider used hand sanitizer after each contact with body fluid.

Plan of Correction: The provider will wash her hands after helping a child wipe their nose.
