Comments:
An unannounced monitoring inspection was conducted on 05/30/2019 from 12:15P.M.-1:45 P.M. At the time of inspection, five children (twelve points) were in the care of two caregivers. Five children's records, one provider record, one caregiver record, one pet vaccine, evacuation drills, first aid kit, emergency supplies, emergency plan, required posted information, and outside play area were reviewed. Children were observed doing during outdoor playtime, lunchtime, handwashing, and naptime. The exceptions to compliance are noted on the violation notice. Description: Based on a review of children records, the provider failed to ensure that all required information was in a child's record. Evidence: 1. Child #3's record did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located. Date of enrollment was 10/15/2018. 2. Child #4's record did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located. Date of enrollment was 03/27/2018. 3. Child #5's record did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located. Date of enrollment was 01/07/2019. 4. Staff verified that the children's records did not contain written authorization for emergency medical care.

Plan of Correction: The provider will have the parents sign the form for authorization for emergency medical care should an emergency occur and the parent cannot be located.
