Comments:
A Subsidy Health and Safety inspection was conducted on 05/17/2019 to review supplemental health and safety requirements. An unannounced renewal inspection was conducted today from approximately 3:20pm-5:20pm. There were 3 children (5 points) directly supervised by 1 caregiver. A sample of 3 children?s records and 3 caregiver's records were reviewed. The children were observed eating a snack and preparing for dismissal. The bathroom of the family day home is being renovated. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review, all caregivers working directly with children had not completed the Virginia Pre-Service Training. Evidence: Caregiver A had not complete the Virginia Pre-Service Training.

Plan of Correction: Training Complete May 20th.

Standard #: 22VAC40-665-400-A
Description: Based on observation, the family day home did not a complete written emergency preparedness plan. Evidence: The emergency preparedness plan was missing lockdown procedures.

Description: Based on observation, the evacuation and relocation procedures were incomplete. Evidence: The evacuation and relocation procedures were missing plans to reunite children with parents after the emergency.
