Technical Assistance:
Licensing inspector discussed an AV for the fireplace, and child abuse and neglect training.

Comments:
An unannounced monitoring inspection was conducted today from approximately 9am-10:50am. There were 3 children (6 points) in the direct care and supervision of 1 caregiver. A sample of 3 children?s records, 3 household member's records and 1 caregiver's record were reviewed. The children were observed playing with blocks, listening to music an pretending to cook. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit was missing required contents. Evidence: The first aid kit was missing adhesive tape.

Plan of Correction: I will purchase some tape, I threw the older one away because it was peeling.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination for each child in care. Evidence: Children A, B, and C did not have a records of a physical examination on file.

Plan of Correction: I will ask for a copy from the parents.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain for each adult household member a current report of tuberculosis screening every two years from the date of the first screening. Evidence: Household Member A last complete a tuberculosis screening 01/27/2017.

Plan of Correction: I will make an appointment.
