A announced monitoring inspection was conducted with the provider. Two children were in care total of four points. total points was four. Children were having free playing and getting ready for school. Two files were reviewed. No children were on medications. All required paper work was posted. Standard #: 22VAC40-111-60-B Description: Based on review of two children's records not all were complete. Evidence: Child A with a start date of 9/4/18 was missing the policy number for the child's medical insurance. Plan of Correction: I will ask the parent for the missing infroamtion.
