Comments:
The licensing inspector conducted an unannounced renewal inspection on Friday, May 24, 2019 from approximately 10:00 am to 12:21 pm to determine the provider's compliance with licensing standards. During the inspection, there were a total of 6 children in care totaling 15 points being cared for by one provider. Upon arrival, the children were observed in the dining room eating a snack and interacting with peers and the provider. Later, the children were observed washing hands, taking bathroom breaks, and playing in their designated area of the home. The provider was observed having positive interactions with the children and assisting them throughout the inspection. The areas for childcare, bathroom, kitchen, the indoor and outdoor play areas were inspected. The required posting were reviewed. The first aid kit and non-medical emergency supplies were also inspected and found complete. Violations from the previous inspection were reviewed for corrections. There were no repeat violations during today's inspection. There were five children's records, one provider and two assistant provider's records reviewed. The home does not administer medications or transportation at this time.

The last evacuation drill was documented on: 05/01/2019. The last shelter-in-place drill was documented on 03/28/2019.

Description: Based on a review of five children's records, observation, and interview on 05/24/2019, the provider did not ensure five out of five children's records contained all the required information. Evidence: 1) The licensing inspector observed that the record for Child #1(DOE: 10/01/2017) was missing the parent's signed acknowledgment of the receipt of the information required regarding the family day home's policy for the administration of medications and documentation of the annual review of the child's emergency contact information. The information was last reviewed on 10/01/2017. 2) The licensing inspector observed that the record for Child #2 (DOE: 10/01/2017) was missing the parent's signed acknowledgment of the receipt of the information required regarding the family day home's policy for the administration of medications and documentation of the annual review of the child's emergency contact information. The information was last reviewed on 10/01/2017. 3) The licensing inspector observed that the record for Child #3 (DOE: 03/15/2019) was missing the parent's signed acknowledgement of the receipt of the information required regarding the family day home's policy for the administration of medications and written authorization for emergency medical care. 4) The licensing inspector observed that the record for Child #4 (DOE: 02/20/2018) was missing documentation of the annual review of the child's emergency contact information. The information was last reviewed on 02/14/2018. 5) The licensing inspector observed that the record for Child #5 (DOE: 03/11/2019) was missing an enrollment form with required information. 6) During interview, Provider #1 reported the enrollment form for Child #5 could not be located while the inspector was on site.

Plan of Correction: Per the Provider: I will find missing forms.
