Comments:
An unannounced monitoring inspection was conducted between 10am and 12pm. Upon arrival there were 12 children in care, totaling 28 points and 3 caregivers. A sample of 3 children and all 3 caregiver records were reviewed. Children were observed playing with toys and learning a lesson with a caregiver. Three children were napping.

Violations:
Standard #: 22VAC40-111-60-A
Description: Based on a record review the licensee did not ensure all required information was maintained for each enrolled child. Evidence: Child #1, enrolled on 06/28/18, did not have documentation of 1 of the 2 required emergency contact information.

Plan of Correction: The following corrective action was provided by the licensee during the exit interview: A second emergency contact will be obtained today and documented in the record.

Standard #: 22VAC40-111-240-A
Description: Based on observation the licensee did not ensure areas and furnishings of the family day home, inside and outside, were maintained in a safe condition. Evidence: The swing set in the front yard had screws with more than 2 threads protruding from the top of the A frame.

Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The screws will be shaved down so no more than 2 threads are exposed.

Standard #: 22VAC40-111-280
Description: Based on observation the licensee did not ensure potential poisonous substances were stored in areas inaccessible to children and away from food. Evidence: There was a large bottle of floor cleaner on the floor in the kitchen next to large bottles of water.

Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The floor cleaner will be moved to the utility closet.

Standard #: 22VAC40-111-320-A
Description: Based on observation the licensee did not ensure all small electrical appliances were unplugged. Evidence: There was a toaster that was plugged into the electrical outlet in the kitchen.

Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The toaster was unplugged during the inspection.
