Comments:

An unannounced renewal inspection was conducted from 10:30 am - 12:45 pm. There were four children present for 7 points. I observed children busy with coloring and playing together. Standard #:	22VAC40-111-830-A
Description:	Based on a review of records today, there was no documentation of fire drills or shelter in place drills since August.
Plan of Correction:	We have not had regular attendance, and I had to close because of having the baby. I will practice fire drills every month.
Standard #:	22VAC40-111-210-A
Description:	Based on a review of records today, the provider did not complete any hours of annual training as required.
Plan of Correction:	I will take the classes online.
Standard #:	22VAC40-111-240-A
Description:	Based on observation during today's inspection, there were some areas that were not maintained in safe condition. Evidence: The play area has some splintering accessible on the wooden deck. Above the area, a large plastic covering has cracked and broken areas on it. There were some splintery areas on the shed in the backyard.
Plan of Correction:	These will be fixed. I will talk to my husband.
Standard #:	22VAC40-111-250-A
Description:	Based on observation today, there were two accessible cables in the children's play area today. Evidence: The two cords from the television were loose and accessible at child height.
Plan of Correction:	These will be put away from children.
Standard #:	22VAC40-111-430-A
Description:	Based on observation today, there was no barrier to the bottom of the stairs in the child care area. Evidence: There was no gate attached at the bottom of the stairs.
Plan of Correction:	We took off the old one but will replace it.
Standard #:	22VAC40-111-500-C
Description:	Based on observation today, the stationery swing set has obstacles in the fall zone. Evidence: The swings are less than six feet from the perimeter of the fence.
Plan of Correction:	We will not use or remove it.

Description:	Based on a review of records today, the form used for the Sworn Statement does not meet current requirements. Evidence: The provider and Household Member 1 and 2 have Sworn Statements that do not include all required information because they are written on a form no longer considered to be current.
